Frequently Asked Questions

We encourage customers to make an appointment to decrease any extended wait times as walk-in appointments are not guaranteed.

Please bring a government issued ID and your insurance information if applicable.

We provide affordable, transparent cash pricing for key health center services for local customers, regardless of insurance status. These services include primary care, labs, X-ray, counseling, dental and hearing.

We suggest you always call your carrier to confirm individual plan details and coverage. Generally, if you’re already on a Medicare PPO plan, you can transition your care to Family Medical Health at any time by scheduling your first appointment. If you are on an HMO plan you should contact your carrier and give them the name of the Family Medical Health provider you’d like to transition your care to. Call us at 888-222-4444 to find the provider nearest you.

If you don’t already have a Medicare Advantage plan, you’ll need to select one that is in-network with a Family Medical Health provider. We accept plans from United Health Care, Humana, Blue Cross Blue Shield, Centene, Cigna and others. Call us at 888-222-4444 to find the provider nearest you and learn about available services like care coordination, in-home health assessments, and remote patient monitoring.

Family Medical Health Centers are full-service health centers that provide access to comprehensive healthcare and accept many insurances, Medicare and Medicaid. Our state-of-the-art facilities are open six days a week and offer primary care, labs, x-rays, behavioral health, dental and hearing services, all in one convenient facility with telehealth coverage available every day of the week.

If you’re newly eligible for Medicare, you can sign up for Medicare 3 months before your 65th birthday, and 3 months after you turn 65. If you enroll before your birthday, coverage usually begins the first day of your birth month. There is a penalty for enrolling late, so enroll in that seven-month period. Some people get enrolled automatically—those under 65 with certain disabilities, or those already receiving Social Security benefits. Visit https://www.medicare.gov/ to see if you’ll be automatically enrolled.

If you’re already enrolled in another Medicare plan, open enrollment is from October 15th through December 7th every year. During this time, you can change, enroll in or add additional Medicare coverage. Changes take place January 1st of the following year. You don’t have to re-enroll every year, you just have the chance to make changes during this time period. However, plans do change from year to year, so it’s important to read the info your plan sends you—especially the Evidence of Coverage and the Annual Notice of Change. To make sure you’re getting coverage that is a good fit for you & that you can afford, review your monthly premiums & benefits.

If you’ve experienced a life event that impacts your coverage, there’s a Special Enrollment Period so that you can make changes. For instance, if you move to a new area with different plan options, you may be able to switch plans for two months after you notify your carrier. Or, if you’re no longer eligible for Medicaid, you may be eligible to make changes for two months after losing coverage. For more information on special enrollment periods, visit: www.medicare.gov.

To enroll, you’ll need your original birth certificate, W2/tax forms and proof of citizenship (passport, social security card, etc).
